Enhanced Care Management, or ECM, is care coordination for eligible Medi-Cal members. Start your assessment through the official ECM form.
Before you apply
Read these steps before you start. They keep your referral correct.
1. Enter your details
If you are applying yourself, enter your own details. Do not use someone else's information.
2. Check the referral source
On page 2, the referral source must say Reconnected Resources. Leave it if correct. Type it if blank.
3. Submit and continue
Finish the assessment at the official ECM site. Keep a record of your submission.
